Have tried .mov, .wmv, .mp4.
Access denied seems to be permission issue or is there settings from my hosting company that would stop me?
I checked all folders and files for permissions.

Local Video Folder set to images/videos

I am having the same issue. I uploaded a .mov video and the put in the tag code of {mov}Art-Attack-Final-websmall{/mov}. It is enabled and I have the folder path correcting to /public_html/images/stories/video. The video is just still not showing up. I set the size of the player in the plugin nad made sure that the spelling was correct on the path. Just can not figure out what I am doing wrong.
