Using flv files located right on the webserver (not remotely located).

Is there a way to start the video with a thumbnail (jpg for example) instead of starting with black?

I'm curious about this as well.  Is there any ideas on how to do this?

I haven't got a response, but hopefully at least the next version of all video will do this.  In the meantime, apparently the joomla extension moseasymedia will do that:
