Contact form module not showing on author page was created by Dan Houston

I have a menu that points to authors. When you click through each author, the have a respective form. All are set up identically, except for some reason, two of the author's forms are not showing up. I'm trying to see what's different about the two but can't figure it out.

Here is the page with all the authors:

The first two author's forms are not showing up when you click through. All the other author forms seem to be fine. Any thoughts?

I assume you need menu items connected to the related form modules, have you checked that?

I sure have. I feel like I've handled all the basic trouble shooting. Don't know why it's showing on most except this specific two when they are handled by the same menu item.

Each contact form module has to be assigned to each respective menu item. You do this on the module's menu assignments. Have you double checked?

The language on all - both working and not working - says default.

Make sure the language is set to all for BOTH modules AND menu items AND the items themeselves & their categories.

We figured it out. It was an issue with sh404SEF. The incorrect non-SEO-friendly link was default for the SEF url for some reason. there was like 14 duplicates on one of these...
