I've set up my K2 and everything works fine. In K2 members post comments which require Admin approval to show on the Front End. However I wish to place a Admin only comments that will always show to the top of the original Item. So only an Admin can contribute this comment and it always shows to the top.

How can I do this?

Simply enable "Enable inline comments moderation" in the K2 Parameters (Comments tab). When an admin posts a comment, they can publish it from the frontend directly.
