I am experiencing the same issue on my site with sh404SEF and pagination. I have 10 pages of items and 1-4 resolve to page-1.htm and and 5-10 resolve to page-2. So I guess it really is broken. For the moment I will probably make hardcoded forwards like @Aaron Wilson did... hardly optimal. Is this a problem with K2 or with sh404SEF... it seems like the whole limitstart thing is pretty Joomla universal.
